Legionella is a Gram-negative, rod-shaped, aerobic bacterium that can be found in natural and artificial water systems. Conditions such as suitable temperatures, water stagnation, sediment accumulation, and biofilm formation can create a favorable environment for the bacterium to multiply within water systems.<\/p>\n
The risk associated with Legionella is not limited to the presence of the bacterium in water. One of the main routes of transmission of Legionnaires\u2019 disease is the inhalation of very small water droplets containing Legionella in the form of aerosols. Therefore, the design of the water system, the way it is used, and its potential to generate aerosols are important factors in risk assessment.<\/p>\n
Legionella infection may be more severe, particularly in older adults, smokers, individuals with chronic diseases, and people with weakened immune systems.<\/p>\n
Which Water Systems Pose a Risk for Legionella?<\/b><\/p>\n
In Legionella control, particular attention should be given to hot and cold water systems, showerheads and faucets, hot water storage tanks, cooling towers, spas and hot tubs, humidification systems, and other water systems capable of generating aerosols.<\/p>\n
In addition, decorative and ornamental fountains found in facilities should not be overlooked as potential Legionella risk points, particularly where water is continuously recirculated, temperatures increase, biofilm can form, or aerosols are generated.<\/p>\n
Conditions such as temperature, water stagnation, sediment accumulation, inadequate maintenance, and biofilm formation may create an environment that supports the growth of Legionella within the system.<\/p>\n
The History of Legionella and Examples from T\u00fcrkiye<\/b><\/p>\n
The name Legionella originates from an outbreak that occurred following the American Legion convention held in Philadelphia, USA, in 1976. During the outbreak, 182 people became ill and 29 died. Subsequent investigations identified a previously unknown bacterium, which was later named Legionella.<\/p>\n
Cases of Legionnaires\u2019 disease and environmental Legionella positivity have also been reported in T\u00fcrkiye. In 2009, five confirmed and one probable case of Legionnaires\u2019 disease were identified at a newly opened 600-bed hotel in Alanya. In another study, Legionella species were detected in the water systems of 11 out of 52 hotels investigated. These examples highlight the importance of regular monitoring, particularly in hot water systems and complex water installations.<\/p>\n
Legionella Control in T\u00fcrkiye<\/b><\/p>\n
In T\u00fcrkiye, Legionella control is carried out within the framework of the Regulation on the Procedures and Principles for the Control of Legionnaires\u2019 Disease, dated 13 May 2015 and numbered 29354, together with the relevant control programs of the Ministry of Health.<\/p>\n
Depending on the characteristics of the facility and identified risk points, important stages of the control process include monitoring water systems, determining appropriate sampling points, collecting environmental samples, and performing laboratory analyses for Legionella.<\/p>\n
In addition to inpatient healthcare facilities, accommodation facilities, and businesses with complex water systems, Legionella risk should also be assessed in production and commercial facilities that contain cooling towers, shower systems, hot water storage tanks, humidification systems, water-based industrial cooling systems, or ornamental fountains.<\/p>\n
Not every air-conditioning or ventilation system presents the same level of risk. From a Legionella perspective, systems that use water, circulate water, or have the potential to generate aerosols are of particular importance.<\/p>\n
Why Is Sampling Important?<\/b><\/p>\n
Reliable Legionella analysis does not depend solely on the laboratory testing stage. Collecting the sample from the correct risk point, using an appropriate sampling method and suitable containers, maintaining proper transport conditions, and ensuring sample traceability are all critical to the reliability of the analytical result.<\/p>\n
Depending on the characteristics of the risk point, swab samples for biofilm assessment may also be included in the testing plan in addition to water samples.<\/p>\n
The Absence of an Inspection Does Not Mean There Is No Legionella Risk<\/b><\/p>\n
The fact that a facility has not previously undergone an official inspection for Legionella does not mean that there is no Legionella risk within the premises.<\/p>\n
In facilities with systems where water is stored, circulated, or converted into aerosols, assessing existing systems, identifying potential risk points, and keeping records of maintenance, monitoring, and necessary analytical testing are important components of effective water safety management.<\/p>\n
For this reason, Legionella control should not be considered only as an activity carried out during inspection periods, but rather as an integral part of the facility\u2019s risk management and hygiene program.<\/p>\n
